Defeat the Mayhem Warmonger to receive this.

In the northwest corner of the upper floor of the chapel, there is an open sarcophagus. Interact with it to obtain the key. Squeeze through the gap downstairs and repair the steps with the Time Rune to reach the upper floor.

On the Mayhem Warmonger. Drop down from the path to the north to reach the open door on the north side of this building, which is open, to find him.
