Speak to Eva after unlocking enough slots on your Crests by using 
Memory Lockets, she will upgrade your 
Hunter Crest. There are two tiers of upgrade, but the exact number that you need to unlock each tier is still not 100% confirmed.
By this point in the game, you will likely have enough to unlock the first tier when you first reach Eva.
You must find at least 6 of the 7 Crests to obtain the second tier. We had a total of 31 slots unlocked across all our Crests when we were offered the second upgrade.
