As pointed out, the main draw of the Chocobo Hot & Cold mini-game is to find Chocographs, which basically function as treasure maps. After digging up a Chocograph at one of the locations, you will need to be riding Choco and then “equip” that Chocograph via the in-game menu. Note that you have to equip the Chocograph in order to actually dig up the treasure, although the only hint you will receive is a picture and a little blurb about it, but not much more than that.
The treasures will come in two different forms, which will be actual equipment/items/cards and the other will be new forms for Choco. These forms will grant Choco new abilities, increasing the terrains he can traverse, not only allowing access to more Chocographs, but also new areas where you can dig for items and even new areas in previous locations. The same hints from Choco (the Kwehs) will be shown when hunting for Chocographs, like with digging for items, so you will know when you are close. Of course, all 24 Chocographs will be listed here, along with screenshots showing you the location of each treasure on the map and in the game.
Chocograph 01 - Streamside

You will find this treasure on the Mist Continent, not too far from where the Chocobo’s Forest is located. Go to the bridge to the west of it and follow the river until it empties into the ocean. On the west side of this is a beach and you will find the treasure pretty much where the beach ends near the river.
Chocograph 02 - Between Mountains

This one is located in the area of the Mist Continent, where Qu’s Marsh is found. Go to the Dragon’s Gate, the one exit of Lindblum, and follow the mountains to the west. Eventually, you will reach a small mountain and become sandwiched between the two. This is where the treasure will be located.
Chocograph 03 - Uncultivated Land

Despite being able to dig up this Chocograph early, you won’t be able to actually get to the treasure until you either have the Mountain Ability. Start off at Treno and go down into the valley nearby, locating the river. At this spot, you should be on the eastern side of the river, which is where you want to be. Locate the marshy textured land, with the portion that’s shaped like a “C.” The treasure will be somewhere on that portion.
Chocograph 04 - Healing Shore

You will find this treasure in the area where Cleyra and Burmecia is, on the Mist Continent. North of Cleyra will be a gap in some mountains, with a beach nearby. Facing that gap from the beach side, you will find the treasure around there; it’s a little bit more on the grass than the beach. This will net you the Reef Ability, allowing you to cross shallow waters, but only accessible from the beach.
Chocograph 05 - Abandoned Beach

On the Outer Continent, there will be some chocobo tracks just north of the Qu’s Marsh there. From those tracks, head directly south until you come to the beach on the shoreline. The treasure will be in the middle of the beach, but more on the actual land.
Chocograph 06 - Cold Field

This treasure is located on the Lost Continent, the frozen one in the northwest portion of the world map. You won’t be able to reach this until you have access to the Blue Narciss. Find the town called Esto Gaza and locate the shore where you can land to the east of it.
Chocograph 07 - Forgotten Lagoon

From the previous Chocograph, head south by southwest, into the waters, until you reach the northern side of the western continent. If all you have is the Reef Ability, then follow the shallow waters to the east and when you reach the dead end, search around there to find the treasure.
Chocograph 08 - Faraway Lagoon

You will find this treasure on the western side of the western continent. If you only have the Reef Ability, find the tracks on the one island off the western side of the western continent, then use the shallow waters to go as far north as possible. At the northern tip of this section, start heading south on the other side and once you reach the second little island just off the shore, search one of the little inlets there for the treasure.
Chocograph 09 - Abandoned Lagoon

Travel to the Outer Continent (the one in the northeast) and find the beach on the southeastern side; you’ll know it’s the right one if you check the name on the menu screen (Kognish Shores). There will be chocobo tracks just north of the forest in this location, so call Choco. While riding Choco, travel along the coastline, to the west, until you reach the peninsula. There will be a bend in the peninsula and the treasure will be in the little inlet there, on the inside of the bend.
Chocograph 10 - Bird’s-Eye Lagoon

Start off at Qu’s Marsh on the Mist Continent, travelling to the west until you get to the beach. Dip your chocobo’s feet into the waters and follow the coastline southwest, until you can get further out into the water. Pass the island near the coast, then begin heading north until you come to a small, round island. You’re close now, so look west to see the island in the Chocograph picture. You’ll have to go around the island to find the spot for digging.
Chocograph 11 - Small Beach

This Chocograph treasure is found off the southern coast south of Qu’s Marsh on the Mist Continent. Once you’re in the water, follow the coastline south until you see an island with a beach on it nearby. The chest will be right in the middle of the coast, right where the water meets the beach.
Chocograph 12 - Dawn Lagoon

Start off by visiting the Chocobo’s Forest and summoning Choco using the tracks near it. Venture east, while still on land, and enter the water from the last beach you find along the southern coast here. While in the water, continue east until you spot a lone island. The treasure will be on the southern side of this island and will grant you the Mountain Ability, allowing Choco to move over mountains on any of the continents.
Chocograph 13 - Forbidden Forest

Thankfully, there’s a pretty good landmark for this Chocograph treasure, so find the Village of Dali and start from there. West to southwest of Dali will be a forest nestled in the middle of the mountains; the treasure will be in that forest.
Chocograph 14 - Green Plains

On the northern edge of the Mist Continent, look for the beach that is shaped like the letter “U” or resembles a horseshoe. Just south of that beach will be some cliffs overlooking it, so look for the highest one and dig for the treasure up there. The Ocean Ability will allow you to walk on the deeper portions of water, so you can pretty much go anywhere there is water with it.
Chocograph 15 - Dusk Plains

This Chocograph is located on the Forgotten Continent, a place which you normally can’t access until you have an airship, but you can actually get this whenever you’re forced here during the story on Disc 3. Head to where Oeilvert is located on your map and southwest of that will be a forest along the coast. Directly next to this forest will be some plateaus, so you want to stand on the lowest one, in the gap between the other two.
Chocograph 16 - Forgotten Plains

This Chocograph is located on the Forgotten Continent, a place which you normally can’t access until you have an airship, but you can actually get this whenever you’re forced here during the story on Disc 3. Towards the northern part of the continent is a building in a valley; this is Ipsen’s Castle. Travel southwest of Ipsen’s Castle and get on the rock formation here, digging around on the first stretch of it until you find the treasure.
Chocograph 17 - Sea at Dusk

Go to the eastern side of the Forgotten Continent (the western one). There is a very tiny peninsula on the eastern coast, around the middle of it. You know you’re in the right spot when you see some forest on the mainland; search a little ways into the ocean until you pinpoint the treasure.
Chocograph 18 - Ocean

The picture for this is a little vague, since it shows mostly ocean, but it is on the western side of the western continent, close to the long island with a lengthy beach. Go on the western side of that island and northwest from the forest there. It’s hard to pinpoint an exact location, so use Choco’s hints to find the treasure.
Chocograph 19 - Cold Lagoon

This treasure will take you to the Lost Continent, which is the frozen one in the northwest portion of the world map. Find the spot with multiple inlets on the northwestern side, and locate the angled one that’s just north of the forest on the land. This is where the treasure will be found.
Chocograph 20 - Mist Ocean

Before you are even able to dig up this treasure, you have to find all six Chocograph Pieces that are in Chocobo’s Forest and Lagoon. Note that you won’t be able to find all of the pieces until you have access to the Hilda Garde III. The treasure will be on the northeastern side of the Mist Continent, a little out from the middle of the two small islands there. The Sky Ability allows Choco to fly, but he can only take off from/land in forests.
Chocograph 21 - Outer Island

Fly on over to the Outer Continent (with your flying chocobo), which is the one in the northeast. Locate Korua Island off the northeastern coast and land in the forest, then search the regular land to the north until you get the treasure.
Chocograph 22 - Outer Island 2

This one is close to the previous one, since it’s still near the Outer Continent in the northeast. It will be on another small island on the northern side of the Outer Continent. The treasure will be found closest to the beach of the mainland, right on the edge of the forest.
Chocograph 23 - Fairy Island

This treasure will be located on Vile Island, which is off the coast from the Iifa Tree on the Outer Continent. Note that this place is home to some nasty creatures called Yans, which can easily down your party if you’re not prepared. The treasure will be at the base of the mountain on the northeastern side of the island.
Chocograph 24 - Forgotten Island

To find this treasure, locate Qu’s Marsh on the Forgotten Continent (the western one). Just off the coast, near Qu’s Marsh, will be a smallish island with a forest on it. Land on the island and get up on the larger of the two plateaus here; towards the one end, away from the other plateau will be the treasure.
