You can start this quest by talking to the Old Beldame in the Swamp Witch’s Hut area - be sure to ring the bell on her gate or succeed at a [Dexterity 15] check, as failing said check or barging onto her property will result in a fight.
Once you’re talking to the Old Beldame, you’ll need to have picked a Mudleaf, Gloomberry or Dizzyhead, in which case you’ll be able to pick the dialog option [Show the herb you’ve found at the swamp] “This seems to be an unusual herb. Can you tell me what it is?”, which will start this quest. The Old Beldame will task you with bringing her three of each of the aforementioned plants - something you can do without leaving the Swamp Witch’s Hut area, if you’re careful.
Mudleaf: There are only a few of these on the map, and are located in the pass southwest of Tiressia (who herself can be found in the northeastern corner of the map). Harvesting a Mudleaf requires you to pass a [Knowledge (Nature) 27], so this will require a competent character and more than likely some save/loading.
Gloomberry: You’ll find these in the northwestern corner of the map, near where three Giant Slugs prowl. Kill them and you’ll be free to loot several Gloomberries in the area.
Dizzyhead: Located in the poisonous mist in the southeastern corner of the map, you’ll likely need to kill a pair of Giants Slugs and the Scythe-Tree to be able to safely harvest them. As for the poison mist, use Delay Poison to nullify its effects long enough for you to harvest them. If that’s not enough bother, you’ll need to send a perceptive character to do the harvesting, as each plant requires a successful [Perception] check (varies, 18-27) to find.
Once you have three of each, return to the Old Beldame to complete the quest. Along with the standard rewards (see below) you’ll also be able to peruse her wares from now on.
